If you or someone you love is struggling with addiction, the first practical step is knowing what options exist. They are not all the same.

If you need help today

SAMHSA's National Helpline: 1-800-662-4357 (24/7, free, confidential, English/Spanish). They'll connect you to local treatment and support — no insurance required for the call.

Levels of treatment

  • Detox — medically supervised withdrawal. Required first step for severe alcohol or opioid dependence.
  • Inpatient rehab — 28+ days residential. Highest intensity; insurance often covers 7-21 days.
  • Partial hospitalization (PHP) — daily treatment, sleep at home. Step-down from inpatient.
  • Intensive outpatient (IOP) — 9-12 hours/week therapy. Most common starting level.
  • Standard outpatient therapy — weekly sessions.
  • Medication-assisted treatment — Suboxone, methadone (opioids); naltrexone, acamprosate (alcohol). Doubles long-term success rates for the conditions where it's indicated.

Support groups (free, ongoing)

  • AA / NA — 12-step. Spiritual framework. Meetings in every town, multiple times daily.
  • SMART Recovery — secular, CBT-based. Smaller but growing.
  • Refuge Recovery — Buddhist-influenced.
  • Celebrate Recovery — Christian, 12-step adjacent. Most churches have a meeting.
  • Al-Anon / Nar-Anon — for family members of people in addiction.

Supporting a loved one

You cannot make someone choose recovery. What you can do:

  • Get yourself into Al-Anon or family therapy.
  • Set clear, sustainable boundaries (not threats you won't enforce).
  • Don't enable (paying bills, lying to employers, replacing things they broke) but stay connected.
  • Be ready to act fast when they ask for help — the window is often short.
